Great article. The use of "black swan" metaphor makes it really intriguing. Considering the adoption of RAD at an excessively high rate, the mistakes are bound to happen in Production. It is why I believe that all organizations should have a Staging or QA environment. So the release migration stays robust minimizing the possibility of an outage.
